Question: I have 5 different versions of my COLB but have never been able to locate a short form Birth Certificate. All my COLB’s are case proper.
Any advice on where to get a short form would be greatly appreciated.
Answer:
The one I value is the long form COLB from the day I was born. Considering the year, it was handwritten. This one confirms a baby was born.
They do now have a long form for the Person/LLC but it’s very basic with ‘mother’ and ‘father’ name but no details about the hospital or nurses or physicians.
I got mine via Vital Chek. I received one or more of the corporate ones but finally figured out the settings/selections to get the long-form.
The one I ordered from the county was the corporate birth certificate.
The key is not use any certificate where the given name and family name are placed beside each other, such as John Smith. Whether upper or lower case is either not important or of secondary importance, IMO. On my COLB, it lists my given name separate from my family name. It may say first name and last name in the fields, but I just don’t want the two names beside each other.

I didn’t think to look at how the name fields are displayed. But the COLB for the baby has them separate. Also, the signature of the attending physician is there, as well as parents’ info, and my given name appears with proper character accent ie, ä.
It’s this document appropriate to use for proof of nationality?
